Our office reviews the circumstances of young people who are seriously injured or who pass away and were involved with the child intervention or youth justice system. These reviews are designed to improve the lives of young people by identifying ways to enhance services and supports, leading to systems improvements and better outcomes for young people and their families.

Individual investigative reviews of each young person’s circumstances include information about the services and supports the young person received, findings, and themes noted. These reviews are publicly posted to our website within one year of being notified of that a young person has been seriously injured or passed away. We analyze this information and, on an annual basis, compete a larger public report that explores the themes identified over that year and makes recommendations to government.
